Home care visits
Home care visits give you or your relative a helping hand at the times of day you need it most, from a 30-minute check-in to several calls a day. A familiar carer supports your routine so you can stay safe, comfortable and independent in your own home.
Who home care is for
Adults of any age who want to keep living at home but need some regular support, including people recovering after a stay in hospital.

What support is provided
- Personal care: washing, bathing, dressing and continence care
- Medication reminders and prompts
- Preparing meals and drinks, and help with eating
- Help getting up in the morning and ready for bed
- Light housework, laundry and keeping the home safe
- Support to get out and about, and to appointments
Each visit follows a care plan we agree with you, so your carer knows how you like things done, from how you take your tea to which jobs matter most. Visits can be as short or as long as you need, and you can add more as needs change.
How home care begins
- Tell us what is happening: Call us or send the form. Let us know which parts of the day are hardest, such as mornings, mealtimes or bedtime.
- Free home assessment: We visit to talk through routines, health, mobility and medication, then agree how many visits you need and when.
- Meet your carer: We introduce your carer before the first visit, so you know who is coming to your door.
- Visits begin and are reviewed: We review the care plan regularly and can add, reduce or move visits when your needs change.

How long is a home care visit?
Visits range from a 30-minute check-in to longer calls for personal care, meals or getting out. We agree the length and timing of each visit with you at your free assessment.
Can home care start after a hospital stay?
Yes. Tell us the expected discharge date on the form or by phone and we will do our best to have support in place when the person comes home.
Other services you might combine with this
Home care visits can be combined with night care for support around the clock, or with companionship and home help for extra company during the week. If needs grow, live-in care is another way to stay at home.
